How It Works, Results, Pain Level & How Long It Lasts

One of the questions we hear most often in lifting consultations these days is, "How is XERF different from Thermage?" Since XERF is a relatively new device, there isn't much information available yet, and many patients are curious. Today, I'll walk you through how XERF works, who it's a good fit for, and the principles we follow at ORTA Clinic when designing each treatment.
What is XERF?
XERF is a non-invasive, monopolar radiofrequency (RF) lifting device that combines two frequencies — 6.78MHz and 2MHz — to deliver energy at different depths of the skin. By raising the temperature within the skin, it induces immediate collagen contraction, followed by collagen remodeling over the next one to three months, which can improve skin elasticity and sagging. It is a medical device approved by Korea's Ministry of Food and Drug Safety (MFDS).
XERF belongs to the same family as existing monopolar RF devices (such as Thermage), but its defining feature is the dual-frequency system, which allows the energy delivery depth to be adjusted across three levels: Shallow, Middle, and Deep.
Results and their duration may vary from person to person depending on skin condition.
Even with the same device, satisfaction with the results depends on who designs the treatment and by what standards. At ORTA Clinic, we follow these principles.
Possible side effects: As with any medical procedure, XERF may cause temporary redness, swelling, or a sensation of heat, and in rare cases burns or pigmentation, depending on the individual. Please consult thoroughly with your medical provider before treatment.
Q. How many XERF sessions do I need?
A. Visible change can be expected from a single session, and results last about one year (individual variation). Depending on the degree of elasticity loss, we recommend regular sessions every 6 months to 1 year.
Q. When will I see results?
A. Tightening from collagen contraction appears immediately after treatment, while the main improvement in elasticity develops gradually over 1–3 months as collagen remodeling progresses.
Q. Should I choose Thermage or XERF?
A. Both are monopolar RF devices that act on collagen in the dermis. XERF's distinguishing features are its dual-frequency system with three adjustable depth levels and its three-stage cooling. The right device differs depending on skin thickness, where the sagging is, and pain sensitivity, so we recommend deciding after a diagnosis.
Q. Does the treatment hurt?
A. With its three-stage cooling system and real-time temperature monitoring, XERF is generally performed without separate anesthesia, feeling like gentle warmth. Pain perception varies by individual, however — if you are sensitive, let us know before treatment and we will adjust accordingly.
Q. Can I wash my face or wear makeup the same day?
A. Yes. As a non-invasive treatment, both washing your face and wearing makeup are fine the same day. Just avoid hot saunas and strenuous exercise on the day of treatment.
